These questions are used in every review of a collective member at Free Geek. You can use the beginning blurb, too.
The following are basic questions that will be in any review. Some of them might be redundant due to xxx's profesional goals feedback section, but please answer anyway. There's also a section for positive and "room for improvement" comments on these items at the bottom. Please fill out those sections, too.
- 1 = never/poor
- 3 = about half the time/so-so
- 5 = all the time/absolutely wonderful
Arrives on time or calls if going to be late: 1 2 3 4 5
Is efficient and makes good use of time: 1 2 3 4 5
Uses the phone, socializes, and takes breaks at appropriate times: 1 2 3 4 5
