Recently while going through modems we found:
PCI 3Com that is a hardware modem, worked great without any goofing around.

I took a big box of PCI modems from the card sorting room and counted them by manufacturer. Here's what I got:
37 Conexant
28 US Robotics (mostly AD1807JS)
13 3Com
9 Lucent/Agere
8 PCTel
5 Rockwell
4 Motorola
3 ESS Teledrive
3 Analog Devices AD1806JS
2 NEC
2 Cirrus Logic
2 Smartlink
1 Creative DS1820
1 Netodragon (?)
1 Ambient MD5628D
116 total
manufacturers marked in bold have known working Linux drivers. Of those, only Lucent/Agere and PCTel are currently shipped by FreeGeek. The ESS modems were taken to the store; after doing some research I discovered that the both the Smartlink and all four Motorola modems are supported by the Smartlink driver (a.k.a. "slmodem"). More work will be required to confirm this and to see how this might work at Freegeek.
