Employees must be regularly schedule for at least 24 hours to qualify for the following:
INSURANCE
- Employees can pursue their own Medical & Dental plan and be compensated $42/mo
- This is considered taxable income
MEDICAL
- Open enrollment period happens once a year in November and closes in December
- Qualifying events exist that allow enrollment outside of the open enrollment period
- e.g., Marriage, Child birth
- 3 Available Plans
- Employee contributions are outlined in the CBA
- Deductible
- Employees pay up to the total amount of the deductible per year, before coverage occurs on certain services
- Base plan: $250
- Co-pay
- Employees pay a flat fee for specific services
- Base plan: $20
- Co-insurance
- Employees pay a percentage of fees for specific services
- Base plan: 20%
- Max Out-of-Pocket
- Employees will pay no more than this fee for specific services
- Base plan: $2000
- Member Services can help to explain the above features, as can the HR Coordinator
- Enrollment will begin coverage on the 1st of the following month
- Leaving the organization will provide coverage until the end of the current month
- Free Geek offers an Alternative Care Rider (Co-pays for alternative health services)
- e.g., Massage therapy, acupuncture, naturopathy, etc.
DENTAL
- Same open enrollment as Medical
- 1 dental plan
- No deductible
- No payment for preventative care
- Co-insurance
- Employees pay a percentage of fees for specific services
- Base plan: 20%
- No orthodontics
- Annual Maximum
- $2000
- Enrollment will begin coverage on the 1st of the following month
- Leaving the organization will provide coverage until the end of the current month
FLEXIBLE SPENDING ACCOUNT (FSA)
- Open enrollment occurs in December and closes in January
- "Use it or Lose it"
- Can be used for qualified medical and dependent care expenses
- Annual cap on contributions to be used for medical expenses = $2000
- Annual cap on contributions to be used for dependent care expenses = the LESSER of $5000 or spouse's income (if married filing taxes jointly) or $2500 (if married filing taxes separately)
RETIREMENT
- Enrollment can occur any time, becoming active on the next paycheck
- Free Geek will match up to 3% of an employee's gross pay
- Max for 2012:
- $ 5,000 Employee
- $12,000 Free Geek:
- Withdrawals before retirement age will incur a tax penalty
- You should be able to roll this IRA into another IRA
- Unsure whether or not you can roll another IRA into this one
- We got a guy who can help to work out unique scenarios
WORKER'S COMPENSATION
Occurs automatically upon incident, requires filing a report
UNPAID LEAVE
- OFLA/FMLA
- 12 weeks
- Military leave
- Extent of active duty
- Random/Miscellaneous
- Up to 30 days at management's discretion
BREAKS
BU Staff will have breaks facilitated by their Direct Supervisor, as per the Organization Chart
Requests for changes in scheduled breaks are explained in the next section
SCHEDULE
- How to request Schedule changes
- Go to http://schedule.freegeek.org from any internet connection
- Click 'Request Schedule Change' near the top of the page
- From here you have the following options:
- Request a meeting
- Request a vacation
- Request schedule history corrections to stop hours logging reminders
- Request other schedule changes
- Fill out the fields; under notes, try to include as much information as possible
- Ideally, all request will be submitted 10 business days in advance
- How to call in Sick
- Call the "Sick" phone (971-295-4542). Leave a voicemail or a text message.
- E-mail paidworkers@ explaining when you'll be gone, when you expect to be back, and what coverage needs to occur
- Try to provide as much advanced notice as possible
- Our PTO covers both vacation and sick time
