Broken commitment chains

At each meeting we write down who has commits to accomplishing a given task. These commitments are read back at the next meeting to see if progress has been made. Unmet commitments might be given to a new owner or continued on to the next meeting.

If someone neglects to carry an unmet commitment forward it is often forgotten.
